Introducción y objetivos: La fibromialgia (FM) es un síndrome crónico que cursa con dolor, fatiga, estado de ánimo alterado y problemas de sueño y cognitivos. No se conoce la causa de la FM, pero las alteraciones en el procesamiento nociceptivo podrían subyacer al cuadro sintomatológico. Además, la presencia de acontecimientos vitales estresantes (AVEs) predispone a la aparición y agravamiento de los síntomas y dificulta la respuesta al tratamiento. Aunque no se ha encontrado un abordaje plenamente eficaz, el entrenamiento autógeno (AT) podría modular la activación del sistema nervioso y reducir los síntomas, incluso en las pacientes que presentan AVEs. Método: 120 mujeres con FM participarán en un ensayo clínico longitudinal. Aleatorizaremos a las participantes en 3 grupos (grupo AT, grupo control y grupo natural). Antes, después y a los 6 meses evaluaremos los síntomas de la FM y la actividad cerebral en reposo. Además, evaluaremos el efecto predictor de los AVEs en relación con la respuesta al AT. Resultados esperados: Esperamos que el AT sea eficaz en el alivio de la sintomatología y disminuya las alteraciones en la activación cortical. Esta mejoría se mantendrá al menos 6 meses. Los AVEs predecirán la respuesta al AT en los síntomas más relacionados con el estrés. Conclusión: La evaluación de la eficacia del AT en el tratamiento de todos los síntomas de la FM controlando el efecto de los AVEs podría suponer un avance en el tratamiento de este cuadro en pacientes resistentes a los tratamientos convencionales.

The cause of FM is unknown, but alterations in nociceptive processing may underlie the symptomatology. Additionally, the presence of stressful life events (SLEs) predisposes individuals to the onset and worsening of symptoms, complicating the response to treatment. Although no fully effective approach has been found, autogenic training (AT) could modulate nervous system activation and reduce symptoms, even in patients experiencing SLEs. Method: A longitudinal clinical trial will be conducted with 120 women with FM. Participants will be randomly assigned to one of three groups (AT group, control group, and natural group). Symptoms of FM and resting brain activity will be assessed before, after, and six months post-intervention. Additionally, the predictive effect of SLEs on the response to AT will be evaluated. Expected Results: We expect that AT will be effective in alleviating symptoms and reducing cortical activation disturbances. This improvement is anticipated to last for at least six months. SLEs are expected to predict the response to AT in stress-related symptoms. Conclusion: Evaluating the efficacy of AT in treating all FM symptoms while controlling for the effect of SLEs could represent an advance in the treatment of this condition in patients resistant to conventional treatments.
